Post-Orthodontic Gingival Recession: Periodontal Perspective and Management

Lecture Description
Not a few patients appear to present post-orthodontic gingival recession, which causes unpleasing esthetics, hypersensitivity, and anxiety. From the dentists¡¯ side, most post-orthodontic gingival recession is related to unfavorable baseline situations and some predisposing factors. However, many patients tend to view the recession as a post-orthodontic complication. Moreover, gingival recession can happen unexpectedly after some years after orthodontic treatment. From a periodontal point of view, the untreated gingival recession may lead to further attachment loss and be susceptible to periodontal inflammation, as demonstrated in a recent systematic review. Periodontal plastic surgeries can be applied to treat gingival recession. However, the post-orthodontic gingival recession seems to be more challenging to manage due to the combination of the following: i) insufficient keratinized tissue, ii) shallow vestibular depth, iii) bony dehiscence or fenestration, iv) partial or complete loss of interproximal papilla, v) high frenular attachment, and v) suboptimal tooth position/angulation (if there is a relapse after orthodontic treatment or if previous orthodontic treatment was not successful). Especially, Asian patients have a thin gingival thickness, making surgical management more difficult. This session will cover a periodontal view of post-orthodontic gingival recession and how to manage the recession through a brief literature review and clinical cases.
